* Onsite & Remote Support customers are required to have a valid credit card on file. Balances will automatically be charged to the credit card on file. I authorize the above-named business to charge the credit card indicated in this authorization form according to the terms outlined above. NC will attempt to install all supplied drivers for the internal hardware on the computer. NC will also run all necessary Windows Updates. For an additional fee NC can install other customer supplied software (office suite, antivirus, games etc.).

Network Concepts is not responsible for ANY lost data. It is essential that the customer have backups of all necessary data. Network Concepts will NOT backup any customer data (unless arranged in advance with the technician additional charges apply). All Windows licensing is the responsibility of the customer. NC is not responsible for ANY lost data. The customer should have backups of all essential data and programs. Some infection removal can cause Windows and/or other installed software to stop working. Some virus/spyware/Trojan infections cannot be removed with a simple antivirus/antispyware scan. The ONLY way to be sure that an infection is removed from the PC is to erase the hard drive completely and reinstall the Windows Operating System. All Windows licensing is the responsibility of the customer. Cost of software not included. Please have your discs, product key information and any account credentials readily available. Technician will transfer data from 1 USB compatible PC to another or will back up data to the device of your choice (external hard drive, CD or DVD) This service does not include moving programs or migrating an operating system. All storage media is the property of the customer. Software/hardware not included. I authorize Network Concepts technician(s) to perform repair work on my electronic device. I understand that Network Concepts technicians have been trained to perform work on electronic devices but Network Concepts is not an authorized service dealer. Further, I agree to release, indemnify, and hold Network Concepts from liability for any claims or damages of any kind or description that may arise from any repair work performed on my electronic device, unless it is cause by severe negligence of Network Concepts or its agent. I understand that Network Concepts is not responsible for any data loss, which may occur as a result of work done on my electronic device. I also understand that I have the option to, and I am responsible for backing up the device before allowing any repair to be performed on my device in the event of any data loss and hardware or software failure. I understand that repairs or technical support performed by Network Concepts may void manufacturer warranties, especially in the case of iOS device repairs. Network Concepts and its affiliates do not assume any liability or warranty in the event that the manufacturer warranties are voided but may, at its sole discretion, offer its own warranty on the parts and/or services performed.
